I think there is a bug in the algorithm that picks which card is drawn immediately after an epidemic card is drawn. In almost every game I have played thus far (and sometimes multiple times within one game) a epidemic card is played, a city is drawn, and then the first card drawn for the infection stage is the very same city that just got hit with an epidemic, leading to an automatic outbreak every time. It never is the 2nd or 3rd city drawn, always the first city. The odds of this happening so many times just due to chance are very slim

I came to the very same conclusion of OP in my games, and I agree that almost everytime it's the very 1st card. Last game it was the 2nd for the first time in my games, but probably that could have been a "legit not bugged" draw.

I talked about this in chat and everyone replyed that it was just bad luck. After these 2 games all agreed with me :P
So I'd check the algorithm just to make sure it's not really RNG making fun of us (with statistics that don't make sense). :P
I have checked the algorithm and didn't see any issue.
I have to remember you that after an Epidemic, cards previously played are shuffled ON THE TOP of the deck. So the odds to get the same card twice are not so small.

c'est un bug que l'on retrouve très très souvent (environ 2 fois sur 3), après une épidémie c'est la même ville qui ressort et explose. En jeu de carte normal c'est pas possible comme probabilité
Je vois ce bug signalé depuis longtemps et non encore traité ... C'est bien dommage
